SMS consent & opt-in
Exactly how consent to receive text messages is captured — the disclosure language shown to patients, a sample intake form, and the consent clause practices agree to.
Consent is captured offline, at the point of the relationship — not on this website. Practices consent in their subscription agreement; patients consent during their clinic's intake process. Patient opt-in — sample intake
Patients opt in through their clinic's patient-intake workflow. The illustration below shows how the SMS consent appears to a patient at intake: a standalone, unchecked checkbox with a full disclosure. Consent is never pre-checked, and is never bundled into an unrelated agreement.

Patient opt-in — exact disclosure language
The disclosure text below is the template Simpliphi provides to every practice for use at intake. Practices replace the bracketed placeholder with their own name.
Before any patient-directed message is sent, the practice must record that this consent was captured for that patient. The record includes who consented, when, and by what method. Patients may withdraw consent at any time by replying STOP; opt-outs are honored across the platform.
Practice opt-in — exact consent clause
Practices (our direct customers) consent to receive operational SMS as part of their subscription agreement with Simpliphi Technologies. The agreement contains this specific SMS clause:
Consent by a practice is recorded, with a timestamp, at the time of subscription.
